About the role: Support all efforts that ensure the successful delivery of Care Navigation at our client sites that have purchased the Care Navigation product.


Essential Functions:

  • Responsible for subject matter expert support to Wellness Center, operations, and product teams in areas of Care Navigation/referral coordination related IT platform(s) and vendor relationships.
  • Support the implementation of new lines of service by working with Health Strategy Associates to ensure implementation schedules are continuously updated and adhered to during the implementation process.
  • Proactively monitors metrics across dedicated book of business clients to identify trends related to potential workflow/process issues and work with Epic team to create and support workflows that deliver optimized referrals for members, providers, and clients.
  • Responsible for standardizing Care Navigation practices across the enterprise, including onboarding and training, documentation, workflows, and reporting and updating SharePoint page with most current information for Field Team members. 
  • Disseminates Care Navigator communications and feedback to key product stakeholders to inform workflow and/or process improvement.
  •  Identifies team member development needs, including training, support, or continuing education, and conducts these sessions accordingly.
  • Develops job aides and reference tools (including CN SharePoint) for Care Navigators to utilize for day-to-day support (works with CN supervisor to make any necessary centralized workflow/process adjustments)
  • Attend and lead community meetings as part of team member engagement and best practice sharing initiative.
  • Support all Premise Health team members and operations managers in a participatory, positive, and supportive manner. 
  • Attends Quarterly Strategy Meetings, and/or other client facing discussions as needed. 
  • Support operations in subject matter expert duties including, but not limited to, interviewing key Care Navigation candidates, staff orientation, training, and ongoing development. 
  • Firm understanding of Care Management Line of Service and ability to support CM Product activities if needed.
  • Performs other related duties as assigned by the AVP of Product.


Job Requirements:

  • Registered Nurse
  • Registered Nurse licensed in state of residence/practice preferred
  • 5+ years of relevant work experience
  • Experience working in Epic and data reporting.

  • Knowledge of adult learning practices and change management
  • Strong understanding of data systems and ability to interpret data as it relates to Product success
  • Strong computer skills in Epic, Microsoft Word, Excel, PowerPoint, and Web/Internet-based programs 
  • Strong verbal and written communication and marketing skills 
  • Strong presentation and time management skills
  • Advanced computer skills in database software such as Microsoft Excel, PowerPoint, Word, & SharePoint preferred
  • Advanced working knowledge of Epic documentation preferred

For individuals living in California, Colorado, Illinois, Washington; as well as, for individuals living in or reporting to New York State only, Premise Health is required to include an estimate of the salary and benefits for this role. While a number of factors influence salary, our estimated California, Colorado, Illinois, Washington, and New York compensation is $89,000 - $109,500. Please note, this is a general guideline and your experience qualifications, geographic location, and other factors will be taken into consideration.
