As a Full-Time Centralized Care Manager Registered Nurse (RN), you'll provide virtual care within our Digital Wellness Center (DWC). This Care Manager RN role will support the Beyond Weight program. Anticipated work schedule will be Monday-Friday, 8:00am-5:00pm CST. Active compact RN license, care management and EPIC experience required. Virtual experience a plus. This role also requires a dedicated, private workspace and high-speed internet. Position may include up to 10% travel.
What You'll Do:
- Develops personalized treatment plans using provider care plans and member preferences using a coach approach and the pillars of Lifestyle Medicine.
- Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion.
- Assesses member’s educational/clinical/social needs, prioritizing care based on elicited information from the member, Epic, and data analysis.
- Collaborates with the full scope of clinical team members to arrange for further medical treatment when needed.
- Provides support and tools needed to help members take control of their health.
- Improves health outcomes and better management of any chronic condition.
- Lowers healthcare costs while improving outcomes.
- Identifies emergency situations and provides care within legal scope of practice.
- Provides condition specific self-care education to client employee enrolled in the various condition management programs.
- Serves as member advocate and acts as facilitator of Care Team meetings.
- Collaborates with health center staff to coordinate and promote health and wellness programs.
- May require other duties as assigned.
What You'll Bring:
- Currently licensed Registered Nurse in state of practice including Compact RN license.
- Current certification AHA or ARC Basic Life Support for health care providers.
- Current Wellness Coach Certification from Wellcoaches® OR equivalent International Coach Federation-accredited program certification (must be obtained within 24 months of hire).
- Bachelor's degree (BSN) from four-year college or university preferred.
- At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.
- Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES).
- Experience with Electronic Medical Records.
- Knowledge of internet software, spreadsheet software and Word Processing software.

Premise provides its reasonable and genuinely expected range of compensation for this job of $73,600.00 - $92,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
