We are currently looking for a remote/virtual Full-time Centralized Care Navigator to join our Digital Wellness Center team. This position will provide coverage within the Central Time Zone. Anticipated work schedule will be Mon-Fri, 8am-5pm CST. Epic experience is required, and referral coordination experience is strongly preferred. This role also requires a dedicated, private workspace and high-speed internet. 


What You'll Do: 

  • Care/Benefit Plans: 
    • Works with the member, primary care provider, supervisor and other members of the health services team (EAP, Case Managers, Fitness, TPA, etc.) to identify and prioritize member's health care goals.
    • Serves as the subject matter expert of clients’ benefits plan design and all partners that utilize it.
    • Navigates client networks and providers and interpret information to patients and their families.
    • Coordinates information flow of benefits between dental, medical and other healthcare providers within the medical community.
    • Fosters communications and coordinates benefits with insurance companies with the goal of creating a smooth patient centered experience.
    • Communicates and schedules appointments with specialists on behalf of members.
  • Member Engagement:
    • Conducts health center outreach.
    • Collaborates with virtual team and onsite client teams to improve member engagement from goal planning through execution and after-action review.
  • Referral Management:
    • Develops understanding of Epic's referral and scheduling capabilities, and available tools.
    • Documents all encounters and follow up plans in Epic.
    • Develops understanding of where member populations live and work, as well as high-value providers and facilities in key geographies.
    • Proactively builds relationships with community specialists, alongside other health center team members, to educate them on Premise and our expectations, facilitate streamlined referral access and information sharing.
    • Works in collaboration with Virtual Primary Care team members and care providers both in the facility and in the community including but not limited to: behavioral health services, disease care management specialist, social workers and community-based organizations to help the member maintain bidirectional communication and achieve optimal health outcomes.
    • Interacts with Premise providers to understand referral needs.
    • Interacts with members via multiple modalities (phone, email, video, etc) to identify appropriate referral target(s).
    • Assists members in scheduling appointments for clinical/medical and other community-based social services and assists with arranging transportation as needed to ensure access to services.
    • Works with community specialists and members to schedule appointments and ensure bi-directional notes transfer.
    • Follows up with member post-specialist visit, ensure member and specialist notes return to Premise, and coordinate ongoing care needs as appropriate.
    • Remains in close coordination with central Connected Care+ team as we evolve our strategy, leveraging new tools and processes
    • Adheres to timeframes and quality standards regarding completion of member assessments, reassessments, care plans, and documentation (including case notes, updates, consents, etc.).
    • Maintains positive and productive personal presentation with internal and external contacts, ensuring all engagements with staff/coworkers, members, and community partners are professional, courteous, and respectful.
    • Is familiar with the Premise Health policies and procedures to assure quality assurance and quality improvement are properly implemented as needed.
  • All other duties as assigned.
  • May include up to 10% travel.


What You'll Bring: 

  • High School diploma/GED.
  • Member Engagement: 3+ years working in corporate wellness/health promotion or with health system logistics/scheduling.

While a number of factors influence salary, our estimated California, Colorado, New York, New Jersey, Minnesota or Washington compensation is $21.54- 26.92/hr.
