We are currently looking for a remote/virtual Full Time Centralized Care Team Manager RN, to join our Digital Wellness Center team. The scheduled hours are Monday-Friday, 8am-5pm CST.
- Leadership experience within the Centralized Care Team is required (DWC)
- Virtual care management & Beyond Weight experience required.
- 50-state licensure and existing compact RN license required
- This role requires a dedicated, private workspace and high-speed internet
What You'll Do
- Acts as subject matter expert to support centralized teams, DWC operations, and product teams.
- Provides effective leadership mentoring and coaching to the team to deliver highest level of service and increase the value of Premise to clients.
- Coaches, develops, and mentors direct reports with an emphasis on building career paths for professional growth.
- Conducts formal performance appraisals for all direct reports and develops action plans for continued development and performance improvement to include progressive discipline and corrective action, when necessary, to ensure high levels of job performance and compliance with company policies.
- Partners with the recruitment team to effectively recruit, screen and hire direct reports.
- Manages payroll activities for direct reports to include timekeeping, approval for time off requests, and other administrative functions.
- Supports the implementation of new lines of service by working with DWC operations, centralized teams, and Health Strategy Associates to ensure successful implementation of new clients.
- Proactively monitors metrics across all centralized care support products to identify trends related to potential staffing needs, capacity planning, and workflow/process issues.
- Evaluates product efficiency and recommends workflow adjustments to align with product demand as needed.
- Disseminates communications and feedback to key product stakeholders to inform workflow and/or process improvement.
- Resolves problems by clarifying issues; researching and exploring answers and alternative solutions; implementing solutions; and escalating unresolved problems.
- Develops job aides and reference tools for centralized teams to utilize for day-to-day support (works with CN/CM supervisors to make any necessary centralized workflow/process adjustments)
- Assists in development of program services and workflows to meet client and program needs.
- Attends Quarterly Strategy Meetings, Care Team meetings, and/or other meetings as needed.
- Has a firm understanding of centralized care support products and ability to support CM activities if needed.
- Performs other related duties as assigned.
What You'll Bring
- Bachelor's degree (BSN) from four-year college or university (preferred).
- Registered Nurse licensed in state of residence/practice; compact license preferred.
- 3+ years of relevant work experience.
- 3+ years leading a team.
- Experience in Condition Management, Care Management, Care Coordination.
- Experience working in Epic and data reporting.

Premise provides its reasonable and genuinely expected range of compensation for this job of $94,000.00 - $119,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
