As a Full-time Centralized Care Team Supervisor RN, you'll provide virtual care within our Digital Wellness Center (DWC). The scheduled hours are Monday-Friday, 8am-5pm CST. This role will serve as a working supervisor, with up to 50% of the time dedicated to member-facing Care Manager responsibilities. Experience within the Centralized Care Team is required (DWC). Virtual care management required; Beyond Weight experience strongly preferred. Existing compact RN license required. This role requires a dedicated, private workspace and high-speed internet.
What You'll Do:
- Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion
- Assesses member’s educational/clinical/social needs, prioritizing care based on elicited information from the member, Epic, and data analysis
- Documents touchpoints and progress into the electronic medical record
- Collaborates with the full scope of clinical team members to arrange for further medical treatment when needed
- Works closely with client’s vendors as appropriate to support member’s needs
- Identifies emergency situations and provides care within legal scope of practice
- Assists in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interprets study results, identifies limitations and barriers and recommends alternatives
- Acts as facilitator of Care Team meetings
- Supervises daily Care Manager operations, providing oversight to ensure program guidelines and clinical procedures are met.
- Troubleshoots issues to completion or escalate to manager
- May develop or assist in development of program services and workflows to meet client and program needs
What You'll Bring:
- Experience within the Centralized Care Team (DWC) required
- Active Compact Registered Nurse (RN) license required
- Current hands-on certification AHA or ARC Basic Life Support for health care providers
- Current Wellness Coach Certification from Wellcoaches® OR equivalent International Coach Federation-accredited program certification (must be obtained within 24 months of hire)
- Bachelor's degree (BSN) from four-year college or university (preferred)
- At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.
- Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plus

Premise provides its reasonable and genuinely expected range of compensation for this job of $86,000 - $96,500 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
