As a Full Time Virtual Primary Care Family Nurse Practitioner (FNP), you'll provide virtual care within our Digital Wellness Center (DWC). Work schedule of 4x10 shifts may vary within the following hours: 7am-8pm CST, Monday-Sunday. Schedule will include weekends and holidays and may be subject to change based on business need. Multiple state licensure will be required (up to 25 states). EPIC experience required; Occ Health experience preferred. Must be able to see ages 2+ and have a minimum of 3 years' experience as a provider. Candidates must reside in a compact nursing state and have high speed internet along with a private office space. 

What You'll Do: 

• Provides quality evidence-based medical care that is routinely provided in a primary care setting.
• Provide medical consultations including the diagnosing and treatment via the phone or video for Premise Health’s patient population.
• Provides referrals both internally and externally to appropriate medical resource or specialty.
• Identifies emergency situations and provides care within legal scope of practice and as defined by Premise Health privileging process.
• Maintains medical records using EMR.
• May require other duties as assigned.

What You'll Bring: 

• Master's degree (MSN) required.
• Licensed as a Registered Nurse and Nurse Practitioner in state of practice.
• Nationally Certified as a Family or Adult Nurse Practitioner.
• Current Certification in AHA or ARC Basic Life Support for medical provider is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
• Appropriate certification to write prescriptions under the authority of the Medical Leader, if allowed within state of practice.
• At least three years of post-residency clinical experience.
• Experience in a primary care, urgent care or emergency room setting. (Occupational healthcare experience may apply in some clinics).
• Prior telehealth health experience required.

Premise provides its reasonable and genuinely expected range of compensation for this job of $98,000.00 - $130,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors. 

For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0769 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
