Registered Nurse (RN), Virtual Primary Care

As a Full-time Virtual Primary Care Registered Nurse (RN), you'll provide virtual care within our Digital Wellness Center (DWC). Anticipated work schedule will be Monday-Friday, 8:00am-5:00pm CST. Schedule is subject to change based on business needs. Minimum of 5+ years RN experience to include primary care and chronic condition care management required; virtual experience within Premise Health strongly preferred. Active compact RN license and EPIC experience required. Must have high speed internet and a private office space. 


What You'll Do: 

  • Triages VPC members and operates at the highest level of licensure to ensure efficiency in the VPC center
  •  Assists with closing out member appointments to include lab/referral/flowsheet orders, scheduling follow up appointments, etc. 
  •  Consistently reviews provider in basket results and contacts members for follow up per provider directives within a timely manner. 
  •  Coordinates with MES and providers for clinical paperwork (FMLA, etc) final review/sign off. 
  •  Confirms prescription refills and passes to provider for sign off.
  •  Supports outreach and clinical conversations when rescheduling missed appointments. 
  •  Runs health maintenance reports to identify attributed members due/overdue for care. 
  •  Assists with closing care gaps by coordinating appointments and continuous care as needed. 
  •  Supports a proactive, multidisciplinary team approach directed toward prevention, education, and health promotion.
  •  Collaborates with the full scope of clinical team members to arrange for further medical treatment when needed.
  •  Assesses member’s educational/clinical/social needs, prioritizing care based on elicited information from the member, Epic, and data analysis.
  •  Serves as member advocate/resource and establishes supportive relationship with members.
  •  Documents accurately all elements of care provided. 
  •  Identifies emergency situations and provides care within legal scope of practice.
  •  Maintains member privacy and confidential member information.
  •  May require other duties as assigned. 
  •  Additional skills that may be required: 
  •  Designs interventions with the member to improve outcomes
  •  Coach effectively virtually using video and phone
  •  May assist Operations in assessing and maintaining reports, evaluating clinical program(s) & conducting ongoing evaluation of the effectiveness of the programs
  •  Develops and presents educational services for associated persons or departments as appropriate
  •  Comfortable with using Excel


What You'll Bring: 

  • Compact RN license required. 
  •  Current certification AHA or ARC Basic Life Support for health care providers
  •  Bachelor's degree (BSN) from four-year college or university preferred. 
  • At least five years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension. 
  •  Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES).  
  •  Certified in an ICF-accredited coaching program preferred. 
  •  Understanding of data analysis in a healthcare environment.
  •  Knowledge and experience with Electronic Medical Records.

Premise provides its reasonable and genuinely expected range of compensation for this job of $36.54 - $45.67 per hour. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.

For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
