Population Health Coordinator

We’re looking for a tech-savvy Population Health Coordinator to work remotely while living in the Orlando, FL area. You’ll support patient outreach and care coordination to help improve health outcomes.
Schedule
Monday–Friday, 8:00 a.m.–4:30 p.m.
Ideal Background & Skills
Bilingual in Spanish or Creole strongly preferred
Medical Assistant or front-desk/health-clinic experience
Proficient with Excel and PowerPoint; comfortable with technology and data
Strong communication skills and understanding of clinical terminology
- Utilizes verbal and written communication skills to develop rapport and trust with members
- Performs member outreach via telephone, secure messaging, and/or other approved communication methods
- Monitors internal work queue and other reports to contact and schedule members where appropriate
- Documents outreach touchpoints and relevant notes into the electronic medical record
- Consults with other members of the healthcare team to solve problems and seek advice
- Maintains member privacy and confidential member information
- May require other duties as assigned
- Additional skills that may be required:
- Collaborates with client, digital wellness center and care managers to coordinate and promote health and wellness programs
- Comfortable using Excel
- High School Diploma or GED required; Associate’s Degree or equivalent work experience preferred
- Completion of a Medical Assistant training program preferred

Premise provides its reasonable and genuinely expected range of compensation for this job of $18.40 - $23.00 per hour. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
