As a Full Time Registered Nurse, Case Manager (RN), you'll provide care to client employees and their dependents in our Health Center located in Robbinsdale, MN. The schedule is Monday-Friday 7:30a-4:30p.
About the role: Provides clinical, casework, and group work services by performing the duties listed below.
Essential Job Functions:
- Assesses the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual
- Coordinates assessment, treatment planning, and return to work processes with the employee, providers, and employer
- Assesses and evaluates cases and prepares and implements treatment and case management plans using benchmarking to guide practices and timelines
- Coordinates and provides care that is safe, timely, effective, efficient, equitable, and patient-centered
- Counsels clients, assisting them to identify and resolve problems and make effective use of resources
- Maintains collaborative working relationships with other community providers to develop specialized services as required
- Acts as liaison between client employees, providers, and agency or institution
- Assists clients in finding ways of adjusting to changes in physical capabilities that will allow for a life/work style as comfortable as the situation will permit.
- Promotes quality and cost-effective interventions and outcomes
- Provides consultation to staff on specific cases
- Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
- May perform duties telephonically as well as conducting on-site visits
- May require other duties as assigned
Education:
- Certificate from an accredited Nursing School or Associate Degree (A.S.) required; Bachelor's Degree (BSN) from four-year college or university preferred.
- Currently licensed Registered Nurse in state of practice required
- Current hands on training in AHA or ARC Basic Life Support for health care providers is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
- Case Management (CCM) Certification, AANC – CM, American Academy of Case Management (AACM), COHN-S/CM, Certification in Occupational Health Nursing, Certification in Emergency Nursing, or Certificate in Case Management preferred.
Experience:
- At least three years’ experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic.
Knowledge and Skills:
- Knowledge of Internet software, Spreadsheet software and Word Processing software.
- Knowledge and experience with Electronic Medical Records preferred.
- Knowledge of workplace health and safety concepts and OSHA regulations preferred.

Premise provides its reasonable and genuinely expected range of compensation for this job of $78,400.00 - $98,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
